Home Health Care

Garden City Hospital understands that home is where you are most comfortable and where healing truly happens. Home health care offers individuals, families, independent living communities and elevated care facilities a variety of skilled and personal care services that provide extra assistance to all ages. Our services enable clients to remain in their homes, retaining their sense of pride and independence. In many cases, family members may live far away from a loved one they are caring for. Home health care bridges the gap by providing dependable care, enabling your loved one they are caring for to remain healthy and safe at home. United Home Health Services is a Medicare-certified, CHAP-accredited home health agency. Services Offered

Medicare Certified Skilled Services: Medicare certified skilled services, paid for by most insurance, provide skilled assessments and treatments by trained and licensed clinicians.

Private Care Services: Personal Care services, both standard and advanced care, are provided by trained caregivers under the direction of licensed clinicians. Although often not covered by most insurances, personal care services are a VA covered benefit.

Patient Eligibility for Medicare Home Health Services

To be eligible for Medicare certified skilled services, a patient must be “confined to the home” (homebound). What is homebound? See below for a quick view of eligibility as defined by the Centers for Medicare and Medicaid Services (CSM). For more information, please visit: htts://

To be eligible for Medicare home health services, a patient must have Medicare Part A and/or Part B and:

  1. Be confined to the home;
  2. Need skilled services;
  3. Be under the care of a physician;
  4. Receive services under a plan of care established and reviewed by a physician
  5. Have had a face-to-face encounter with a physician or allowed non-physician practitioner (NNP).

Care must be furnished by or under arrangements made by a Medicare-participating Home Health Agency (HHA).

Patient Eligibility: Confined to the Home

An individual shall be considered “confined to the home” (homebound) if the following two criteria are met: 

Criteria One
One Must Be Met:

  1. Because of illness or injury, need the aid of supportive devices such as crutches, canes, wheelchairs, and walkers; the use of special transportation; or the assistance of another person to leave their place of residence.
  2. Have a condition such that leaving his or her home is medically contraindicated.

Criteria Two
Both Must Be Met:

  1. There must exist a normal inability to leave home.
  2. Leaving home must require a considerable and taxing effort.
